Dr. Irina practices via telemedicine only. The patient must be physically located in the state where Dr. Irina is licensed (AZ, CA, ID, WA) at the time of each visit.

Steps to become a patient:

1. Review information on www.drstrelyuk.com particularly the New Patients and Autonomic Response Testing (ART) pages.

2. Click on the Become A Patient button below, fill out the form and submit.

3. Check your inbox for a Patient Portal Registration email and follow the steps to create your Patient Portal account in ChARM. (This email will be sent to you within 24-48 business weekday hours. Please check the spam folder if not seeing an email within this time frame).

4. Fill out the three consent questionnaires (the New Patient Intake and the Medical Symptoms Questionnaire can be filled out later prior to the first appointment). Questionnaires can be found under the Questionnaire section (see yellow highlight to the right).

5. Once done filling out the three consent forms message Dr. Irina via ChARM (see green highlight to the right) to discuss scheduling your first appointment (you can provide a few times and dates that would work well for you).
